Food Allergy Form

To help accommodate you or your child’s medical food allergy, this form must be completed 7 days prior to your stay. Due to our meal planning and food ordering schedule, we are unable to guarantee that we will have meals to meet your medical food allergies if this form has not been submitted in time.

Please only fill out this form if you have a medical food allergy.

If you have questions prior to filling out this form, you can call or email our Food Services and Hospitality Manager. Please note that calls or emails will not be seen on evenings or weekends.
